Medical Conditions
Record current and past medical conditions and history in the Lifecard app.
These may include chronic conditions such as diabetes or autoimmune diseases. You can also include short term conditions in this section, such as Gastro.
Attachments
The attachments section is where you can store any other piece of information relating to your health.
These include:
- Referral letters
- Medical certificates
- Doctor’s letters
- Diagnostic result summaries
- Health summaries
- Discharge summaries
- Ultrasounds
- Images of wounds
- Any other document
You can upload to this section via your camera roll on your phone, or via document upload on tablet or computer. Your healthcare provider can also send you information, this will arrive in your inbox where you can choose to keep the attachment in your profile.
